Chiropractic Billing Challenges That Cause Claim Denials

Chiropractic billing is one of the most heavily audited areas of Medicare. CMS data shows that roughly 30 percent of Medicare chiropractic payments were improper in 2025, most of them because the documentation did not support the service. Medicare only covers chiropractic manipulative treatment codes 98940, 98941 and 98942 for a documented subluxation and every active treatment claim must carry the AT modifier. Leave it off and the claim is denied. Add it to maintenance care and the payment becomes an overpayment that falls under the 60-day overpayment rule and can be recovered later. Medicare maintenance visits require modifier GA and a signed ABN, while excluded services like exams, X-rays, extraspinal manipulation and therapies require GY.

Commercial payers add more complexity: billing 97140 with manipulation requires modifier 59 or XS for a separate body region, while E/M visits require modifier 25 when separately billable. Visit limits, prior authorizations and personal injury claims further delay payments, leaving staff chasing insurers and doctors correcting notes instead of treating patients. The result is more denials, payment takebacks and lost time.

How Our Chiropractic Billing Services Protect Your Revenue

MedLife MBS manages the full chiropractic revenue cycle from inside your EHR, so SOAP notes and claims stay connected with no retyping. Our coders specialize in chiropractic workflows. They check that each note shows the subluxation, the spinal regions treated, the treatment plan and measurable progress before a Medicare claim is released, and they know when a visit has moved from active to maintenance care. Modifier logic for AT, GA, GY, GZ, 25 and 59 or XS is applied claim by claim based on documentation, not habit. Every claim undergoes payer-specific checks, while denials are traced to their root cause to prevent repeat errors. Monthly KPI reviews track clean claim rates, denial trends, AR days and collections. Whether your clinic uses a hybrid cash and insurance model or relies entirely on insurance, services are available as full billing support or individually to strengthen your existing team.

  • Benefits and eligibility verification: Our eligibility verification services confirm chiropractic benefits, visit limits, deductibles and authorization rules before the first adjustment.
  • Chiropractic coding and modifier review: Certified coders assign 98940 to 98943, 97110, 97140, 97112 and E/M codes with the correct AT, GA, GY, 25 and 59 or XS modifiers, and the right subluxation and secondary diagnoses.
  • Documentation compliance review: SOAP notes, treatment plans and re-exams are checked against Medicare documentation rules, including PART elements, so claims hold up if they are pulled for audit.
  • ABN and maintenance care management: We flag visits that are moving into maintenance care, make sure a valid ABN is on file and apply GA or GZ correctly to protect both the clinic and the patient.
  • Claim submission and denial management: Clean claims are submitted daily, statuses are checked every 24 hours and our denial management team corrects and appeals rejected claims quickly.
  • AR follow-up and patient billing: Aging claims, personal injury balances and patient copays are worked by payer and age bucket, and statements go out on a consistent schedule.

Trends Shaping Chiropractic Billing Solutions

Denials are rising. Medicare scrutiny is up. Practices that stay passive will bleed cash. Here’s what to watch.

Medicare Improper-Payment Surge

33.6% of chiro claims face denial risk under CERT audits. That’s $178M in claw-backs—avoidable with the right safeguards.

Modifier Confusion is Costly

Modifier-59 on 97140? GA for maintenance care? Missteps mean delays or outright denials. We handle the nuance so you don’t have to.

Admin Time Is a Killer

DCs lose 28 hours/week to billing tasks. Our automation restores those hours to care time and not a time for paperwork.

Staffing Gaps Mean Risk

It’s harder than ever to hire trained billers. Our dedicated team is pretrained and fills the gap without needing any further training.

AI is Becoming Standard

Chiropractors now expect AI pre-scrub tools. We deploy them to flag errors, guide edits, and boost clean-claim rate.

Shift Toward Hybrid Models

New DCs are opting for cash + insurance hybrids. Our flexible billing adapts to your model so you’re not boxed in.

Frequently Asked Questions

They cover the full revenue cycle for a chiropractic clinic: eligibility checks, coding, modifier review, documentation compliance, claim submission, payment posting, denial management, AR follow-up and patient statements. MedLife MBS also provides a monthly KPI report.

Medicare Part B covers only manual manipulation of the spine to correct a subluxation, billed with CPT 98940, 98941 or 98942. Exams, X-rays, extraspinal manipulation and therapies performed by a chiropractor are not covered by Medicare.

The AT modifier must be added to Medicare chiropractic manipulation claims when the treatment is active or corrective, meaning the patient is expected to improve. Without it, Medicare treats the service as maintenance care and denies it.

Maintenance care is not medically necessary under Medicare, so the AT modifier should not be used. If the patient signed an ABN, add the GA modifier so the patient can be billed when Medicare denies the claim. Without an ABN, use GZ.

GY is used for services Medicare excludes by law when performed by a chiropractor, such as exams, X-rays and therapy codes. It tells Medicare the denial is expected and allows a secondary payer or the patient to be billed.

Notes must show the subluxation, the treatment plan with goals, and at least two of the PART elements: pain, asymmetry, range of motion changes and tissue changes, with one being asymmetry or range of motion. Progress must be measured over time.
